Agreement to Terms

By accessing or using the services provided by Zion Home Health Services, INC. ("Company," "we," "us," or "our"), you agree to be bound by these Terms of Service. If you disagree with any part of these terms, you may not access our services.

Description of Services

Zion Home Health Services provides in-home and virtual therapy services for children, teens, and young adults ages 0-21. Our services include but are not limited to:

• Speech therapy
• Occupational therapy
• Physical therapy
• Behavioral therapy (coming soon)
• Counseling services (coming soon)
• Parent training and support

Eligibility

Our services are available to individuals ages 0-21. For minors, a parent or legal guardian must consent to services and participate as required. You must provide accurate information during registration and maintain the accuracy of your account information.

Appointments and Cancellations

We require 24-hour notice for appointment cancellations or rescheduling. Late cancellations or no-shows may result in a cancellation fee. Repeated missed appointments may affect your ability to continue services.

Payment Terms

Payment is due at the time of service unless other arrangements have been made. We offer the following payment options:

• Private pay (direct payment per session)
• Medicaid - accepted for eligible patients; see our Medicaid page for covered plans
• Superbills provided for commercial insurance reimbursement (out-of-network)
• Commercial insurance is not billed directly; superbills allow families to seek reimbursement

Technology Requirements

Virtual therapy services require reliable internet connection and a device with video and audio capabilities. You are responsible for ensuring your technology meets the minimum requirements for telehealth services.

Confidentiality and Privacy

We are committed to protecting your privacy and maintaining the confidentiality of your health information in accordance with HIPAA and applicable state laws. Please review our Privacy Policy and HIPAA Notice for complete details.

Limitations of Telehealth

While telehealth services offer many benefits, they have limitations including:

• Technical difficulties may interrupt sessions
• Some assessments may require in-person evaluation
• Telehealth may not be appropriate for all conditions

Emergency Services

Our services are not intended for emergencies. If you or your child is experiencing a mental health crisis or medical emergency, please call 911 or go to your nearest emergency room immediately.

Intellectual Property

All content on our website and provided during therapy sessions, including materials, resources, and documentation, is owned by Zion Home Health Services and protected by copyright laws.

Limitation of Liability

To the maximum extent permitted by law, Zion Home Health Services shall not be liable for any indirect, incidental, special, consequential, or punitive damages resulting from your use of our services.

Termination

Either party may terminate the therapeutic relationship with appropriate notice. We reserve the right to discontinue services if these terms are violated or if continuation is not clinically appropriate.

Changes to Terms

We reserve the right to modify these Terms of Service at any time. We will notify you of significant changes by posting the updated terms on our website and updating the effective date.
