Our services are available to individuals ages 0-21. For minors, a parent or legal guardian must consent to services and participate as required. You must provide accurate information during registration and maintain the accuracy of your account information.
We require 24-hour notice for appointment cancellations or rescheduling. Late cancellations or no-shows may result in a cancellation fee. Repeated missed appointments may affect your ability to continue services.
Payment is due at the time of service unless other arrangements have been made. We offer the following payment options:
Virtual therapy services require reliable internet connection and a device with video and audio capabilities. You are responsible for ensuring your technology meets the minimum requirements for telehealth services.

To the maximum extent permitted by law, Zion Home Health Services shall not be liable for any indirect, incidental, special, consequential, or punitive damages resulting from your use of our services.
Either party may terminate the therapeutic relationship with appropriate notice. We reserve the right to discontinue services if these terms are violated or if continuation is not clinically appropriate.
